Understanding patient care documentation

I’ve been noticing how different facilities approach patient care documentation, and it’s a bit baffling sometimes. At my current workplace, we have to log every single interaction, while I’ve heard others only require summaries. Do you think this level of detail improves patient care or just adds unnecessary stress for us CNAs?
