New staffing rule for nursing homes — any real changes

CMS says 2.45 CNA hours per resident day and 24/7 RN coverage are coming — has your facility started adjusting? Ours keeps saying “we’re fine,” but they’ve been begging for doubles since July; is that just us, or are you seeing new hires and schedule shifts?

We were “begging for doubles since July” too until we started posting the PBJ HPRD by shift; turns out nights were 0.3 under the 2.45, so we moved two day CNAs to permanent nights and added a small weekend diff, which steadied the schedule… If your admin says you’re fine, ask for the rolling 4‑week HPRD and the 24/7 RN grid so you can reallocate instead of grinding people down.

Same vibe — management says ‘we’re fine,’ but our midnight holes showed up when we mapped 2.45 by unit instead of building-wide averages. What moved the needle was a small night/weekend differential and splitting nights into 6-hour CNA micro-shifts, plus a 0.6 FTE house-sup RN to meet 24/7 (on-call doesn’t count per CMS: https://www.cms.gov/newsroom/fact-sheets/nursing-home-staffing-standards-final-rule). @travelerrunner5975 would your team let you pilot 6p–12a and 12a–6a CNA slots?

What moved us was adding a 10p–2a float CNA three nights a week to push nights over “2.45” without blowing up days; we hired it as a per‑diem micro‑shift, which kept cost low but can be flaky. Have you tried carving out short night blocks since July?
