Helping PT and tracking wins

Quick example: over two weeks, one of my stroke patients went from 10 ft to 60 ft with the gait belt during PT, and I jot the distance plus assist level in a simple 0700/1500 note so we can show progress and keep them fired up. How are you assisting PT and tracking those small wins in a way that’s safe and easy for a CNA just starting out?

I do the 0700/1500 blurb too, but add device and cue used (FWW + “heel-toe”) so PT sees what worked. > so we can show progress and keep them fired up. How are you assisting PT and tracking Totally with you on the timing; a tiny dry-erase strip on the walker handle with date, distance, and assist keeps the patient and staff seeing the win each lap. Just clear the max distance/assist with PT first — , the min/mod/CG wording gets fuzzy; would your floor allow the walker tag or should we keep it to a quick room whiteboard note?
